Concrete form work

Someone with knowledge of proper form techniques for building forms. Rebar installation, and knowledge of code requirements. Must be able to read blue prints and etc. If you can frame and perform miscellaneous other construction components even better. Must have minimum 5 years in form work, own truck, tools and trade.
